Does anyone have a form for weekly student progress reports avail.

Help! I'm trying to find a simple for for teachers to fill out weekly to
report my child's progress. I can't find one, and before I work through
developing one, I was hoping someone had one that worked and it's easy for
busy teachers to quickly fill out.

Well, I don't have kids, so I sure haven't developed anything like this.
