Does anyone have a good NCR template?

Well, it might be National Cash Register or No Carbon Required, but I
suspect christa is looking for a Non-Compliance Report (or Non-Conformance
Report) format.

Try a Google search for something like

nonconformance report template word

If you're complying with specific requirements from a government body, ask
them for a template or a sample format, because they probably have specific
requirements for the report, too. :-}

And NCR "No Carbon Required" paper was invented by National Cash Register,
was it not? Those were the only things that NCR brought to my mind, in any
case.
