double attachments when sending with outlook xp

I've got a weird problem here, occasioanlly when trying to send an
email with two different attachments, it will send the same attachment
twice instead. This shows up on the receivers end, and also in the
senders sent items. We are using Outlook XP and SBS2000.
Anyone have any thoughts?
