I like the way Word 2000 handled inserted graphics with text boxes around it.
i.e., the graphic would be "in line with text" and the text boxes (used for
call-outs) would by default be "in front of text." You could group them along
with the accompanying arrows and lines and the graphic wouldn't jump around.

It would be nice if this could at least be an option under "Compatibility"
or something optional.

In Word 2002 and 2003, you can set the default text wrapping for
inserted/pasted graphics (it is In Line With Text by default). This does not
apply to AutoShapes (including text boxes), which by default have Square
wrapping, but you can change it to In Front of Text.

You can disable the drawing canvas on the General tab of Tools | Options.

You can set the graphic insert default in Word 2007. Office button - Word
Options - Advanced tab - Cut, copy, and paste section, 4th option listed.

To enable/disable the drawing canvas, look at the 5th option under Editing
options, also in the Advanced tab.

Am I correct in my understanding that the only way you can group the text
boxes around a graphic is to use the drawing canvas (or put everything in
cells within a table)?

I believe this may be true in Word 2007; I don't think it's true in earlier
versions: certainly I have grouped graphics and text boxes in Word 2003, but
they do all have to be in the drawing layer. You can't group text boxes in
the drawing layer (In Front of Text) with an inline graphic.

In Word 2007, the canvas is one of several techniques. If you despise the
drawing canvas, you can work in Compatibility Mode, where grouping works as
it did in Word 2003. Then, when you're done, if you need the file in docx
format, save the finished stuff in Word 2007 format.
