Dropdown List items in an email template?

Hi all,

I would like to create a template that has 4 drop down lists in it (and two
blank text fields) with the resulting email to me highlighting the chosen
item from each list and the free text fiels.

Anyone know how I would do this in an email template?- the link below is an
example of what i would like to have presented in an email - it's the way i
collect information from a website but i also need to give people the option
to email me directly using this template...also means they cant enter
eronoeous information.

I searched everywhere to find this answer a while back. I ended up getting
the new Acrobat 8...problem solved. And it allows you to import the data
into excel or access. Good luck.
