DUPLICATE CONTACTS IN OUTLOOK

I use the motorola smartphone and sync with outlook. Every
time I sync I end up with every cotnact in outlook on my
PC duplicated. Any solutions out there for a) removing
duplicates b) preventing reoccurance?
