Duplicate Outlook and Address issue

I reinstalled Outlook 2003 on new XP install and have a couple small issues.
One is that I now have a duplicate Outlooks on the left which I can't close.

Then when I click to add an email address from my contact list it gives me
the error message that my address list could not be displayed... yadda yadda..

When I click OK it then shows 2 Personal lists to pick from... the first
which is blank and the second has all my names.

Any help would be greatly appreciated.
