Duplicate Outlook and Address issue

I reinstalled Outlook 2003 on new XP install and have a couple small issues.
One is that I now have a duplicate Outlooks on the left which I can't close.

Then when I click to add an email address from my contact list it gives me
the error message that my address list could not be displayed... yadda yadda..

When I click OK it then shows 2 Personal lists to pick from... the first
which is blank and the second has all my names.

Any help would be greatly appreciated.

Whatever you did corrupted your profile and created a ghost PST file. You'll
need to create a new profile and connect it properly to your PST file. Once
you do, you can then configure your Outlook Address Book view correctly:
