Duplicate Schudules Items in Outlook after changing orginal meeting

When I change a scheduled meeting I sometimes end up with the old item as
well as my new meeting in my calendar.

What version of Outlook are you using? Specifically what steps are you using
to change the meeting? Are you the organizer of the meeting? It only
happens sometimes?
