Duplicate tasks, one with "(assignment)" appended, appearing in Resource Assignment view

I can't find much documentation on this issue so I'm hoping someone here
will have a quick answer.

My Project Plan manager reports that in Project Web Access, when she clicks
Resources then selects "View resource assignments" and "All assignments" for
a specific resource, some of the resource's tasks are appearing twice in the
resulting list. One of the items has "(assignment)" appended to the end.
Each item also appears to have different actual hours worked attributed to
them.

As far as I can tell, the only action taken on these tasks recently is that
we republished them in Project Professional (Collaborate -> Republish
Assignments...) to put them under a new manager for approvals, etc

Can someone help me with (1) what the duplication means, (2) what caused it,
(3) how to reconcile it/make it go away?

Are your team members complaining of duplicate task assignments on their
View My Tasks page? If so, this means that someone published a master
project to the Project Server database. It would also mean that your
Project Server administrator has allowed the publishing of master projects,
which is a mistake, because doing so can cause duplicate task assignments.
If this is the case, your Project Server administrator will need to do the
following:

1. Delete the master project from the Project Server database
2. Deny the permission to publish a master project

Let us know.

I believe you will find that resources have been assigned to summary tasks.
You will need to either remove the resources from the summary tasks and
transfer their actual work to the correct task. The republish, overwriting
actuals on resources' timesheets.
