After creating a new outline numbering style "MyOutline", I notice
that it shows up multiple times in the "Bullets and Numbering" gallery
dialog (Format, Styles, Modify, Numbering) thus replacing the Word
defaults.

1) Why does it show up multiple times?
2) How can I clean it up?
3) Why does the gallery come up in the first place, since I already
selected the style I want to edit at Format, Styles, Modify? I assume
the 'real' one is the one that is selected by default, but this is
very confusing.

Before I apply my style in a new doc, selecting Format, Bullets &
Numbering from the menu bar brings up a 'normal' looking gallery w/
the standard Word defaults. After I use my style in the doc, the
gallery shows the duplicate entries (like when I bring it up via
Format, Styles, Modify...

4) Why does it do this?

5) One of the outline options available looks as follows. Where in
the world is the text "Test" coming from. It is nowhere in the style
definition.

a. _____________
i Test _____
_________

All this seems really strange.

By "outline numbering style" do you mean a List Style? If so, I can't help.
But if you're trying to apply ordinary outline numbering to a single style,
you'll be in trouble since you you really need a separate style for each
