E-mail merge

I'm doing a e-mail merge and it works fine but I want to
put a form attached and it does not work. my form is a
template. I can not do this and have the form stay
intact and have it able to be tabbed through and filled
out. Is this something that is not possible or am I doing
something wrong?

Word/Outlook cannot do mail merges with an attachment. You'd need to use a
third party mass mailing program such as the ones listed here:
