I wish to produce a form where the recipient fills out certain fields similar
to a questionnaire. I do not want the recipient to be able to alter the
question only fill out the box or select a dropdown choice. I wish to place
the forms on a disk and give it to our clients who can complete them and
email them to me.

Being new to this I assume Word. However, I will leave it to your expertise.
