email duplicating the file many times over

The end user emailed a file to another end user and the
file kept duplicating itself and on the return email, the
same thing occurred.

I scanned the file/folder and then installed current
update to antivirus (semantic) no viruses were detected,
but the problem continued to happen...Help

I dont know what to do...It is Microsoft Outlook 2002
