Email follow up flag does not always produce Reminder

I use the follow up flag on email regularly. Lately I have noticed that the
reminder does not always come up. I always request a date and time. It seems
like the reminder usually works, just not always. At one time I thought it
might have something to do with setting a reminder that is only an hour or
two away, but I've found that it also happens sometimes with reminders set
for the next day. Has anyone else had this problem? Any ideas on how to make
sure I will always be reminded?
