Email form results based on unique ID of form

I am trying to email the results of a form. Depending on the Type of Request
on the form, the request will go to a specific email address. For example, if
user need "username" then the form results will be emailed to "user1@domain.
If the username need "password" then the form reults will be emailed to
user2@domain.

Any ideas?

Not possible when using the FP Form Handler, you will need to use/process the form via a custom
written server-script form handler, such as ASP, ASP.net, PHP, CGI, etc.
