Email Printing Issue

Office 2003. Outlook. Windows XP. SP3. HP 990 Printer.

When I print email (and this is only true for email), I always get a rarely
used email address printed at the top of the page and a date at the bottom.

I can't seem to figure out how these got added or how to delete them.

Thoughts?
