email results of a form from an email address entered in a text fi

I have a created a web form (survey) and I would like the results of the
survey sent to the email address entered in a text field (the email will
change often and it is too much to create a dropdown list). How do I code the
form to read the eamil text field and send the results of the form to the
email entered in the email text field when Submit is clicked?

You will need a server side language (ASP, .NET, PHP) to actually do this.
