email template and mail merge

I am trying to create an "appointment confirmation" email that will
automatically paste fields from the person's contact info
(name,addr,phone,appointment time) into this email template.

I send these emails 1 at a time, so mail merge doesn't seem like the right
approach.

You could possibly code it in. Try an Outlook Forms group.
