Care to describe what you see a "paperless employment form" doing? The term
means nothing to me.

I should have said Employment Application Form.
My company uses an application that has multiple pages. Some of the
information, such as the name is used in each page. I would like to develop a
PC screen to capture all of the information required or allow the applicants
to fill out the application and then print those pages that require a
signature only and all of the info is captured and stored. The supervisor
would also use this to track application and employment trends such as source,
or how the applicant learned of the job opening.

Well... the details of what information is needed in an employment application
will have some similarities between different businesses, but they'll also
have a LOT of differences. The information you collect when hiring a roofer is
somewhat different than what you need for hiring a PhD chemistry researcher!
In addition, different companies' human resources departments have different
traditions and different information needs.

This surely won't be a template, or a ready-made application suited to YOUR
company. It would be a pretty substantial (but doable) development project to
create one. Or you could Google for commercial packages - surely this is a
common enough need that somebody has written one (which, again, may not be
tailored to your specific needs).
