i have table (Initiation) contains items and percentage for each item
and also i have a form (Checklist) contains the fields from this table . the
checklist in the form should appears in a check Box form.
what i am asking for is when i make a check for each item in the form i want
to give a percentage for this check (the percntage read from the table
Initiation) and store it in other table.
