Entourage only syncs in iCal with one calendar called Entourage. You
have to make sure any entry you want to sync back to Entourage has a
calendar assigned to it as Entourage.

I'd start from scratch using Entourage as the master. Turn off sync in
Entourage prefs. Now turn it on. When you get the popup asking you
what you want to do have Entourage overwrite the Apple apps. Once done
and Addressbook and Ical are populated turn sync off. Now run
Entourage with only one module for sync enabled at a time because if
you have more than one module turned on you'll get a sync mess. Turn
on Calendar and tasks. At the popup choose combine. Evertime from
hereon if you Enter something in Ent it should appear in iCal in the
Calendar Entourage. In reverse, just make sure everything is labeled
Entourage including that on you iPhone (if you want to use multiple
calendars on your iPhone you'll have to use iCal and forgoe
Entourage). When you want to sync another module turn off the active
module and turn on the module you want to sync (one module at a time),
let it sync then switch back to your most active module.

You should have no problem with sync if you do this.
