Entourage Identity Problem

Did you revert to an earlier version of Entourage?? Baybe through Time
Machine?
Maybe you tried out the EWS version (13) and reverted to the regular
one??

In any case, make sure Office is as up to date as possible (12.2.0 with
the 12.2.1 patch)
