Entry appearing twice in TOC

Hello all

The last entry in my TOC is appearing twice, and I can't for the life of me
figure out why. The heading that is being linked to only appears once in the
text, so it's not picking up another heading. Both entries link to the same
place when clicked.

If anyone can help, it would be much appreciated.

If there is a manual page break preceding the heading, delete it and instead
format the heading as "Page break before."
