Error message in Outlook 2007 AND Shutdown error in Outlook

I USED to have Microsoft Access Outlook Add-in for Data Collection and
Publishing installed and functioning (and the Inbox still has a a subfolder
named Access Data Collection Replies).

For some unknown reason, that add-in is no longer functioning correctly and
does not send/receive data as it used to and I can't find where to get the
program to re-install it.

The problem is two-fold - first, anytime I now select any email message to
read, I receive an error message titled 'Custom UI Runtime Error in Microsoft
Access Outlook Add-in for Data Collection and Publishing' with the attached
details stating 'An error occurred when calling the callback:
"Ribbon_GetDCVisible" '

Secondly, Outlook now NEVER closes correctly and I usually get a message
telling me it has not closed correctly and to re-start the program and close
again. Doing that SOMETIMES closes Outlook correctly.

What is happening? How do I correct it to remove the error message and
restore correct Outlook functioning? How do I re-install the missing Add-in
that is not even listed in the Outlook Add-in list?

Has the add-in perhaps been disabled because of the crashes?
