error on submitting a form

I am trying to save a form submittal to a file which then
posts some of the inputted form fields into a confirmation
page. However I am receiving this a page with the
following url:

..../_derived/nortbots.htm

and this error

"The page you are looking for might have been removed, had
its name changed, or is temporarily unavailable."

I created the form and assigned a confirmation page and a
results page but nothing seems to be working.

How about a URL for your form page?

Or, the form HTML if it isn't published to the web.
