estimate request form template

Can anyone suggest or provide a useful excel or word
estimate request template or form that includes all necessary information
for commercial printing ie. quantity, ink, paper, trim size, delivery etc.

FYI - I'm the print buyer sending estimate request to a commerical print
vendor
