Extracting ER Diagram fields from Visio 2003

I am using the Entity Relationship template.

Each shape has "database properties" in Visio 2003. How do I extract
these database properties to a text document or excel spreadsheet using
programming? (.NET preferred)

I would like to get the Columns, Primary Key, Indexes, and Notes.
