failed operation when sending

When testing changes to my custom form I receive
the "Operation Failed" error. This occurs when
the 'send' button is clicked to route the form to the
person specified in the to box. I have been making
changes to this form and in prior testing have never
received this error before. How do I find what failed
and how to fix? Are there settings that somehow got
changed or properties that need to be done differently?
I am at a loss.
I hope this all was understood.

Without knowing what version of Outlook you're using or what kind of form it
is (mail, task, appointment), it's hard to know where to begin.
