Feedback form problem

I am trying to create a feedback form which asks a series of questions
similar to http://www.do-you-comply.com/health_check.html.

On testing it, when a radio button is checked and then subsequent radio
buttons are checked the previously checked buttons become unchecked.

What am I doing wrong?

Use this info = Checkbox Use check boxes for optional items. The site
visitor can select or clear the check box. They can also select multiple
items.
You are currently using = Option buttons Use option buttons (also called
radio buttons) when you want the site visitor to select only one option from
a group. A B C

I found cut and pasted the above for you from FrontPage2002 help section
called "About creating forms"
