File Symbol in Frontpage 2002

In my Folder List mode in FrontPage 2002 I have a page which does not show
my NAVBAR for one page on my web site. I noticed that when viewing the
pages as a Folder List in FrontPage the one page that does not work called
intro.htm shows a graphic icon next to it as a piece of paper with lines on
it and a blue circle with an arrow on it. The pages that do work show a
similar icon but with the Internet Explorer E on the piece of paper. All
files are htm, what is different about this one that does not work. All the
code in both the working and non-working files are very similar??? Make
sense?

Yes I published the whole web page.

Look at the following pages which are ok.
http://acctgrp.com/sites.htm
http://www.acctgrp.com/jobs.htm

Then look at the HOME and Introduction pages which the nav bar is missing.
http://acctgrp.com
http://acctgrp.com/intro.htm

It seems that the links are trying to find a capitalized INTRO.HTM page.
Where can I change this. The file is in lower case? I think the lower case
and upper case reference is what is hurting me when I moved the web site to
a Linux server. All file names are in lower case.
