Filename and path keep printing in Excel headers

I have someone whose Excel printouts keep including the full path and
filename of the spreadsheet he is trying to print. We have checked to make
sure there is nothing in the header, saved the files and yet the header
continues to include this info. New files also seem to be picking this up as
a default.
