My main form is for my Contact table.
It includes your basic data of name, Address, City, etc.
I have a field called ContactPatientRelationship.
If that field is filled in as "self",
i want my sub-form (to input Data for my Patient Table) to automatically
fill in the corresponding LastName Field, Address Field, CityField, etc.
