filtering report from checked boxes on a form

I have a form with checkboxes, where each check box is a value from a table
or a query.
for example. when i check schoolName and SchoolAddress check box, click on
the report preview button, a report shows up with only 2 fields on it
schoolName and SchoolAddress.
My problem is, when i open a form checkboxes are not white nut gray, and
when i check off any boxes and click on the report preview button, i get an
error.
Please help
