finding record if already exist in table

I want my form to seek for the record based on the field "year" and "Period"
if the record exist show the value for the other field (amount,etc..) in the
form if the record does not exist let the form create a new record for the
"year" and the "period" value initialy keyed so I end up with no duplicate
record for "Year" and period.
