Form check boxes disappear

I am using Word 2000 on Windows 2000 SP3. I have a form
that has many form fields and check boxes. When I try to
use the form and check one of the check boxes, the check
box and form field disappear. The document is properly
protected, but it is an older document. I tried copying
and pasting into a new document and all the check boxes
disapear. The other form text fields seem to be intact.
anyone have any suggestions how to restore this document
to original state?

I did not create the form, is there any way I can tell if
these are active X controls?
