Form File Upload

I created a test page with a form that will send me name,
address, phone..etc. plus for people to upload a file to
me. When you fill out the form and attach a file it comes
up with a login dialog box after clicking submit. The
folder on my site where the uploaded file should go has
the permissions "modify". This folder is not
login/password protected. The link is www.promo-
art.net/test_form.htm. Any suggestions would be
appreciated.

Your host need to set write permissions.

Provided your host supports File Upload under the FP 2002 SE, you need to check the instructions at
