Form of address used in cover letter if you have no name

When you are writing to a company and have no specific person to address, do
you write:
Gentlemen:
Ladies and Gentlemen:
Dear Ladies and Gentlemen:
Dear Ladies and Gentlemen,
Dear Sirs:

Which forms or address are currently acceptable?
