Form "Option" stopped reporting via email.

I installed a set of check box options plus other fields
using the Form Wizzard. I have directed the websiteb to
send me an email with the results when the SUBMIT button
is clicked. Three of the four check boxs s report their
status the fourth doesnt state "on" when checked. How do I
repair this? It worked for a while. Thanks for the help

You can not have spaces in a form or field name.
