Form problem when inserting data. (Hit or Miss)

Here's the deal. I have a form that has all of our company's product in
it. When at a customer site I can lookup a part number and cross it to
ours. I then insert this information into another table where I can run
a report for the customer. I need to add a field for customer part
number which I have in both tables. I also added a text box on my form
for this information. It seems to be working fine. The problem I have
is it's hit or miss. sometimes it adds the customer part number and
sometimes it dosen't. can someone tell me what i am doing wrong,
