First, turn off CAP lock.
Second, where are you trying to test the form, your local machine or the
remote server, which must have the FP extensions, and the web must have been
published to the server via FP's http mode, not FTP.
Also the host must have configured the extensions to process email, If this
is on the remote host, have you contacted them regarding this?
