Form results feed to Database problem

I've created a form to be used as a survey and have successfully uploaded it
to our department website. Following the FP steps I also created the Access
Db for the results to feed into, but here's where I've run into a problem.
While I can fill the form out, submit it, and print out the results, an error
message "Cannot update the db," appears and the results are not sent. I've
also received this error message: "Server error: unable to retrieve schema
information from the record source 'Results' in a db string Driver={MS Access
Driver (*.mdb)}; DBQ=URL+fpdb/Master Survey Form-3.mdb. Any ideas on how to
fix this? Thanks for the help!

Filenames with spaces and/or special characters are known to cause a lot of
problems, not just with databases. Try renaming your database to something
like this: msf3.mdb and reimport it into your web site.
