form results missing

I used the Forms Wizard in FP to set up a form. When
tested, sometimes it goes to the thank you link and
sometimes there is an error message. Either way, there
has not been any information posted to the form results
page. My server has FP Server Extensions.

How do I get the form results?

Hi:

First of all, contact your host and ask them what it says in the server's
systerm log, and report that back to us.

1. view your page in code or html view and delete the null onsubmit which looks like this in the
opening form tag:
onSubmit=""

Save the page.

2. Remove the following and place it elsewhere on the page, it is contained within the form tags
but is not part of the form:

This form looks great, but is a testing ground. Please submit your answers and also e-mail your
ratings to (e-mail address removed). Thank you!

Save the page again.
Publish

per Tom W. reply, Cox Cable has no system log to refer to
and is of no help whatsoever.

in HTML, I deleted:

in fact, i have deleted that twice and it keeps coming
back. i saved and published both times. i am getting
my "thank you" page instead of an error message, but i am
still not finding any results. i am looking in
_private formrslt.htm and it always says Form Results
Inserted Here. Once when I tried to submit the form, I
got a blank page that was titled

ballPartyform.htm
