Form results to txt file problem

The problem is that I am emailing results to my Cell phone. I get the name,
phone,date and time just fine.
That crazy message up top on the 1. line has got to go. It makes my phone
readout two pages long. I was about ready to give up then I remembered how
much help I have recieved from newgroups every time I hit the wall.
Help me mr. wizard
