Form that collects report parameters

Can someone please direct me to a sample of an Access db using a form that
collects report parameters? This would be extremely helpful as I am
experiencing Brain Freeze after the holidays. Thank you in advance.

try Rogers Access Library

Look for Choose Report from List - there are 4 sample db's that run reports
from user selections on a form.
