form that sends results to an email

I'm having a terrible time trying to figure out how to make a form that
sends the results to an email. The part that is most difficult is trying to
figure out how to fill out the menus on the Form Properties. I'm looking
for some step by step instructions so I can put the information on this
page: http://www.electsheryl.com/how-you-can-help.htm into a new fresh form
page with a SUBMIT button that will send the contents to an email.
