form will not send

My forms at www.yourchoiceyeovil.co.uk were working fine.
Now when I try to send one I keep getting a message
asking for sign on name and password - this happens from
any computer on the "mailing list for buying form".
Anyone help please ! Simon

Not ! But it has been working for a couple of months
with Lycos - Simon - might this be a problem?

This means that there is a permissions error on the host server. If you're
saving results to a file, then it could be the _private direcytory, or the
file, has lost the ability for anonymous users to write to the location. If
these permissions are correct then it could be the anonymous user does't
have permissions to execute (run) any part of the extensions, thus prompting
the user for a password in order to process the form. Contact the host as
they're probably the only ones who can handle it since it is on the server
end.
