Format Cell

I like to acheive this dayte format in a Cell 27-Mar but also in the
same cell ( add text) i.e "27 Mar (Chq)"

At the moment I can only acheive 27/03/2006 (Chq) though the format is
set to dd-mmm in format/custom

dd-mmm "(Chq)"
