Formatting text in Word tables and forms

In Excel, I can format text to display as a phone nbr. Is there a way to do
this in a Word text input field (form) inside a table?

Sorta. Depends to a certain extent on the version of Word (some don't support
custom formats as well as others). You can usually force it with a macro,
though.
