forms and mail merge fields

Have a form that is 3 up on an 8.5 x11 page. Would like to insert a name, address, city, St ,Zip in a text box on each form. have 110 clients to mail. Am at a loss as to how tho accomplish this 'simple' task.

Set it up as a label merge, defining a custom label three-up on a page.
Don't use text boxes; use paragraph formatting to position the merge fields.
