Since Excel is a cell by cell form, I can't figure out
anyway to design a form in which you have boxes for data
that don't line up underneath each other. If anyone knows
what I am talking about and could help, this would be
great.

The data arrangement would be as shown below:

data field data field
data field data field

The above is one record with its fields on 2 separate
lines and they are not lined up directly below the other.
